    Full Trainer Pack

    Download an editable version of the full trainer pack, then add course and programme information on your computer (or by hand later) and then print ready for use on a course. The full template includes the course information sheet, attendance record, venue risk assessment, trainer report, partial completion record, participant evaluation form and (if required) physical skills participant declaration.

    Partial Completion Record

    This form should be completed by the trainer for each participant that is not eligible for certification for all the modules covered in this course, if any. It will be used to determine the person’s certification.

    Venue Risk Assessment

    This form records the trainer's venue risk assessment prior to the commencement of any physical skills training. It should be completed by the trainer and retained by the Maybo Training Centre.

    Joining Instructions for Physical Skills Training Participants

    This one-page PDF should be issued to all participants of Maybo courses that will involve the practice of physical skills prior to the day of the course. It can be shared with the link, or downloaded and attached to an email, or printed.

    Physical Skills Training Participant Declaration Form

    A printed copy of this form must be signed by each participant at the start and end of all Maybo physical skills courses and retained by the Maybo Training Centre.

    Injury Report

    This form should be used to record all known or suspected injuries that occurred in relation to a Maybo training course. Injuries should be reported to Maybo during the online certification process post-course.
